On the fifth page, on the right-hand side, there is a rectangular black stamp with the text in the original language: «Nr.inwentarza A-1317.», below which is an impression of a blue oval stamp with inscriptions in the original language: «Muzeum imienia Dzieduszyckich we Lwowie» and an impression of a black rectangular stamp with the text in the original language: «Dział przyrodniczy». On the left-hand side is an impression of an oval black stamp with the text in the original language: «Biblioteka Poturzycka W.D.». The page also bears the handwritten inscriptions: «W64, 1990, and 11372».
