Provenance: On the title page, in the upper left corner, the label bearing the original-language heading «Biblioteka Muzeum im. Dzieduszyckich we Lwowie» features the handwritten inscriptions «Sz 14.a, No. 91» and «A-1322.» The handwritten number «817» also appears on the page. Centered on the second page is an impression of a rectangular blue stamp with the original-language text «Nr.inwentarza A-1322.» Below this are impressions of two stamps bearing the original-language text «Muzeum imenia Dzieduszyckich we Lwowie.» The handwritten inscriptions «1984,» «1990,» and «11.376» are also present on the page. Additionally, the page bears the handwritten inscription «Jakubowicz Zacharyasz» and a noble seal impression (a coat-of-arms crest) that directly indicates the noble lineage of the register's author, Zachariasz Jakubowicz.
